Forms & Required Documentaion

You will need to complete and submit the following forms and documentation prior to your Family Visit (student assessment and parent interview). All documentation must be received before an admission decision can be made. Use BCA's Application Checklist below to help guide you through the Admissions process.

Forms to print (download at bottom of page):

• Admissions Application
• Statement of Faith
• Tuition Statement
• Handbook Acknowledgement
• Parent Participation Policy
• Transcript Release
• Teacher Recommendation
• Principal Recommendation

Documentation to submit:

• Recent Student Photo
• Birth Certificate
• Social Security Card
• Current Immunization Record
• Current Vision & Hearing Test
• Special Testing (Relating to the Academic, Social or Health well-being of your child.)
• Special Education &/or 504 Records
• Divorce Decree (if applicable)

Please submit all forms and documentation to BCA's Admissions Office by:

Once all forms and documentation is received, your Family Visit (student assessment and parent interview) will be scheduled. Family Visits are usually one hour, and can be scheduled any Monday or Friday from 9:30 AM to 3:30 PM, with the last available appointment scheduled for 2:30 PM.
